For all outpatient appointments:

Please plan on arriving approximately 20 minutes before your first scheduled appointment to complete your intake information.

For parents of youth:

Please arrive 20-30 minutes before your child's first scheduled appointment to complete all intake paperwork. Both parent and child must be present for initial appointment.
