Home Based Counseling Services
Home based services are provided for children, adolescents and their families across the Twin Cities area. Family Innovations is certified through the Minnesota Department of Human Services as a Children's Therapeutic Services and Supports (CTSS) provider. Home based counseling is available in the metropolitan area, including but not limited to the counties listed below. In order to be eligible for home-based counseling, a child must be on Medical Assistance. Services are available at times that work best for the family, including evenings and some weekends. Family Innovations home-based counselors work closely with the referral source, such as case managers or county social workers, to insure quality care for their clients. Family Innovations works with children from the age of 3 months up to age 18, and with 19-21 year olds under certain conditions.
A comprehensive diagnostic assessment and individualized treatment plan are developed in conjunction with the family. Ongoing individual and family skills, consultation, education and training, and case coordination are provided as intensively as needed to support the child and family.
